Our goal is to be here for you when it works for your schedule with the option of scheduling by appointment with a provider. If memberships are your thing, we've got those too. Membership Policy

1. The first month’s payment is due at time of first session, auto pay each month thereafter.
2. Limited memberships are a month-to-month subscription. Unlimited memberships are 3/month commitments.
3. An Intake and consent form must be completed and filed prior to your session.
4. Any session cancellations must be done twelve hours prior to scheduled appointment time to avoid fees.

All memberships auto-renew monthly. Membership cancellation requires a 30 days notice from the monthly billing date. If you would like to cancel your membership, simply send us an email at info@blue32jax.com

Please note, you can also request to either “downsize” your membership if you don’t need as many sessions each month.
